Job Description
We are looking for an enthusiastic and well motivated individual to join the Integrated Medicine Therapy Team to implement the Early supported discharge pilot scheme.
The successful applicant will be responsible for providing rehabilitation to patients along the unscheduled care pathway focusing on the provision of ongoing assessment and rehabilitation post discharge, in order to reduce the length of stay, reduce unnecessary admissions and readmissions.
The successful candidate will have and be able to demonstrate:
* Experience of rehabilitation within Medicine
* Excellent communication skills
* Excellent organisational skills
A driving licence and/or access to a vehicle is desirable for this post due to the requirement for regular travel.
