Cardiology is a clinically based specialty, which encompasses the diagnosis, assessment and management of patients with cardiovascular disease. This popular specialty is high profile, exciting and demanding. Careful analytical skills are essential and decisive action can be life saving. Most consultants divide their time between inpatient and outpatient clinical duties including coronary care and investigation/interventional procedures. Sub-specialties include Non-invasive cardiology, interventional cardiology and electrophysiology.
The specialty is at the cutting edge of medicine with the rapid translation of academic research to clinical practice. On call duties can be demanding and rewarding. A phone call is often not enough, but a rescue angioplasty may save a life.
Personal qualities should include:
Cardiac physiologists carry out crucial diagnostic and analytical procedures for patients – ranging from babies to the elderly – with known or suspected heart disease.
Cardiac physiologists, have a lot of patient contact, and work as part of a large team providing care to heart patients, including cardiographers, cardiologists (doctors), specialist nurses and operating department practitioners.
Cardiac physiologists assess patients with vital diagnostic equipment during what is often a distressing time in their lives. Cardiologists use the information gathered by cardiac physiologists when making a diagnosis and prescribing appropriate treatment.
Although they are likely to be based in a hospital cardiology department, their job will vary depending on the types of procedures carried out by the hospital. They may monitor heart function during surgery and intensive care, and carry out procedures, including echocardiography or pacemaker implantation and follow up.
Echocardiography obtains moving pictures of the heart to help the cardiologist diagnose any disease in the heart valves and/or muscle.
Pacemaker implantation and follow-up involves taking measurements to ensure that a pacemaker is functioning correctly when a patient is in theatre. After this, they treat patients in follow-up clinics to make sure that the pacemaker is functioning well and the battery life is satisfactory.
They also help with exercise stress testing and may advise whether or not it is safe for a patient to continue with a test.
Most cardiac physiologists are based in hospital cardiology departments – with both in- and outpatients. Others work in GP practices, operating theatres or critical care units.
Traditionally, entry into cardiac physiology was through a training post with on-the-job training, leading to voluntary registration with the Registration Council for Clinical Physiologists.
However, as a result of Modernising Scientific Careers, there are increasingly opportunities to enter this area of healthcare science at undergraduate level through the NHS Practitioner Training Programme (PTP) or through the graduate entry route of the NHS Scientist Training Programme (STP). As a result, opportunities for trainee cardiac physiologists through the traditional route will probably disappear as these new routes become established.
The minimum entry requirement for the PTP is typically a good range of A-C grade GCSEs plus a minimum of two A levels or equivalent including a science subject. However, this is just a guide. Applicants must check with the universities they wish to apply to, to confirm their exact requirements.
For STP training places, a 2:1 in a relevant science degree is the minimum required. Each NHS organisation advertising STP vacancies decides which degrees are relevant.
The cardiac physiologist is the friendly professional face in this high technological environment linking the patient with the diagnostic equipment. Cardiac physiologists often work with patients who are unwilling or unable to co-operate. A calm, confident but sympathetic approach is required to achieve an appropriate outcome for the patient.
For the PTP, entry is through an accredited BSc Healthcare Science (Physiological sciences – specialising in cardiac physiology). These are 3-year programmes which combine academic study with practical experience gained in the NHS.
Students on these programmes do not receive NHS financial support.
At STP level, trainees should search for STP vacancies in cardiac physiology.
For cardiac physiologists who have followed a "traditional" approved training programme, a voluntary register is currently held by the Registration Council for Clinical Physiologists.
Arrangements for registration under Modernising Scientific Careers is still under discussion.
